Megumi has saved Yuji from Sakuna who wanted to take his body after throwing his heart again. Yuji is now getting treatment inside the hospital, he is unconscious and inside his body, he is talking with Sakuna. Sakuna told him not to look inside him without permission he doesn’t like it. Sakuna is sitting on Yuji’s bones in front of the fingers that Yuji has swallowed before. Yuji told him to come down and he will look down on him.
Sakuna said that Yuji is being unfriendly and he replied yes saying that Sakuna almost killed him. Sakuna asks if it is what that is the thanks he gets after fixing his arm earlier. Yuji reply yes saying that Sakuna ripped out his heart after fixing his arm. Yuji asks Sakuna if he is in hell and he won’t be stuck here with him even after death. Yuji attacks Sakuna with the head of an animal that was in front of him and Sakuna jump out of his spot. He then quickly rushes to where Sakuna is firing powerful blows.

Yuji tricks Sakuna by acting like he’s going to attack his face and he hit the bone that Sakuna is standing on. Sakuna notices that Yuji wanted to throw him inside a pool of red blood then he teleports himself behind Yuji. Sakuna told Yuji that he is boring and kicks him on his butt then he falls inside the pool of blood. While Yuji is lying down Sakuna land on Yuji back and sits on top of him. He told Yuji that this is not hell it is his inner domain.
Yuji notices that it is what Megumi was talking about before and Sakuna reveals that they are not dead yet. Sakuna told Yuji that if he accepts his condition he will fix his heart and bring them back to life. Sakuna told Yuji who is complaining that things have changed and in the future, he will see something interesting. Sakuna said he has two conditions one: when he chants Extension Yuji will hand over his body for one minute.
The second one Yuji will forget this promise and Yuji said he has noticed that Sakuna is evil and he is not lending his body again. Sakuna said that he is promising that he won’t kill or hurt anyone during that minute. Yuji said that he can’t believe him and Sakuna said it is not about believing. There is a pact, a geas and if he breaks it he will be punished. After Sakuna explains how Yuji’s powers along with jujutsu work Yuji agrees to his conditions.
Yuji punches Sakuna as revenge and told him that he has conditions too. Sakuna said they can fight to the death if Yuji wins he will do it without conditions. Yuji was ready to fight boosting his confidence and Sakuna didn’t waste time and he defeated him with a single shot. Sakuna fixes Yuji’s heart and he wakes up in front of Satoru, the nurse, and Ijichi. Satoru welcomes Yuji back and they are both happy.
